Five Ways Enterprise AI Projects Fail in APAC — and What to Do Instead

Pattern recognition from 30+ engagements across nine markets. The failures are remarkably consistent.

The five failure modes we see most often — and why they are so predictable.

After thirty-plus AI engagements across Hong Kong, Singapore, Japan, Taiwan, Malaysia, Korea, China, Indonesia, and Vietnam, a pattern has become impossible to ignore: enterprise AI projects fail in the same ways, repeatedly, regardless of industry or market. The technology is rarely the problem. The organisational and process issues underneath are.

Here is what we have actually seen — and what we have learned to do instead.

Failure Mode 1: Measuring success by model accuracy, not business outcome

The project team celebrates 92% accuracy on their validation dataset. Three months later, the CFO asks what changed in the business. Nobody has an answer.

This is the single most common failure mode. Technical teams optimise for what they can measure precisely — AUROC, F1, precision-recall. Business leaders care about case resolution time, cost per transaction, revenue per advisor, staff hours freed. These are not the same metrics, and the translation between them is not automatic.

What to do instead: Before selecting a model architecture, define the business KPI the AI workflow is supposed to move. Design your measurement framework to capture that KPI — not a proxy for it. If you cannot connect the technical performance metric to the business KPI with a clear causal chain, the project does not have a success definition yet.

Failure Mode 2: The pilot that never scales

The pilot was successful. Everyone agreed. And then… nothing. Twelve months later, the pilot is still running, the team is still "reviewing the findings," and the vendor is still on a rolling contract.

Pilots fail to scale for predictable reasons: no executive owner with P&L accountability, no integration into the operational workflow (the AI runs parallel to the existing process rather than inside it), and no funding model for production.

What to do instead: Agree on scaling criteria before the pilot starts. Define what a successful pilot looks like in quantitative terms, and identify the sponsor who has authority to approve the production investment. If that person will not commit before the pilot, the pilot will not scale.

Failure Mode 3: Data ownership is unresolved

Six weeks into the engagement, the team discovers that the data they need is owned by three different business units, two of which have competing internal mandates that would be compromised by sharing. The project stalls while the politics are resolved. Often they are never resolved.

This is especially acute in conglomerate structures common in Japan, Korea, Indonesia, and the Philippines, where business units operate with significant autonomy. An AI project that requires cross-unit data sharing requires cross-unit executive alignment — which is a political project, not a technical one.

What to do instead: Map data ownership and governance before scoping the technical solution. If the data required for the project cannot be accessed within the engagement timeline, change the project scope — not the timeline.

Failure Mode 4: The AI team is not embedded

The AI team delivers a model. The operations team does not adopt it. The AI team does not understand why — they never spent time in the operations workflow. The operations team was never involved in defining what the AI should do.

Technical teams working in isolation from operational teams produce technically correct solutions to the wrong problem. In our experience, the biggest predictor of adoption is the depth of collaboration between the AI team and the frontline users during discovery and design.

What to do instead: Put the AI engineers in the room where the work actually happens — before writing a line of code. The discovery phase should produce a shared problem statement that both teams can agree on. The solution design should include at least one explicit review session with the end users who will use the output.

Failure Mode 5: Governance is an afterthought

The model is live. Three months later, an audit committee asks for documentation on model drift monitoring, bias testing, and retraining triggers. The team discovers that none of these were set up. The model is running without any oversight mechanism.

This is becoming an existential risk as regulations tighten. The MAS Model Risk Management framework, HKMA's AI governance principles, Japan's METI AI Guidelines, and Korea's AI Basic Act all require documented governance processes for AI systems used in regulated activities. A model deployed without governance is a future regulatory incident.

What to do instead: Design the governance framework before deployment — not after. This means: defining retraining triggers, establishing a monitoring cadence, documenting the model card, assigning a named accountable owner, and setting thresholds for human review. If the governance framework cannot be designed within the project timeline, the project is not ready to deploy.

The common thread

All five failure modes share a root cause: treating AI adoption as a technology project rather than an organisational change project. The technology works. The organisation is the hard part. Every engagement we run spends as much time on the organisational side — stakeholder alignment, governance design, operational embedding, measurement frameworks — as on the technical side.

The teams that get this right are the ones where the executive sponsor stays engaged past the pilot phase, where operations leads are involved from discovery, and where governance is designed before the model goes live. The technology, at that point, is almost the easy part.
